Register With The Practice?

Please come to reception where you will be asked to fill in a registration form if you are within the practice boundary of LS28 and LS13.

In order to register with the practice we require you to attend for a short medical. This consists of a questionnaire completed by yourself beforehand and an interview by a nurse. We also need a urine specimen.

The whole medical should take no longer than about 30 minutes.
