Making A Complaint
We try to offer a friendly and efficient service but misunderstandings can arise, or you may be unhappy about an aspect of our service. If you wish to complain then we suggest you talk to the business partner (in
his absence, the office supervisor). A copy of our complaints procedure is available at reception and is also displayed for your information.
Remember, we are always ready to listen.
If your complaint cannot be resolved locally, you may go to Leeds Primary Care Trust. Write or phone your complaint to:
Leeds PCT,
North West House,
West Park Ring Road, Leeds
LS16 6QG
Telephone: 0113 305 9400.
Data Protection
Patient information will only be discussed on a need-to-know basis. The clinicians reserve the right to discuss your clinical case with another clinician. Medical information will not be disclosed to any
unidentified or unauthorised person.
Zero Tolerance To Aggression
Leeds doctors have a zero tolerance agreement which is followed strictly at this surgery. We will not tolerate any verbal abuse or physical violence towards any doctor, nurse, staff member or other patient. Any
patient who is deemed to be abusive or aggressive will be removed from the practice.
Freedom Of Information – Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available.
This scheme is available from reception.
